PLEASE READ THESE TERMS AND CONDITIONS OF USE AND PRIVACY STATEMENT CAREFULLY BEFORE USING THIS APPLICATION.

For the purposes of these Terms and Conditions, "SSB Mobility" means SSB Mobility Company Limited and "Affiliate" means SSB (Bangkok) Company Limited "Hospital" means network of "HEALTH CONNECT" member hospitals.

By using "HEALTH CONNECT" application services and any other related applications created by SSB Mobility ("Application"), you as an application user agree to be bound by the following Terms and Conditions of Services ("Terms and Conditions"). If you do not agree with all the Terms and Conditions, please do not use this Application. By using the service, it is considered to be an expression of intention and acceptation of terms and conditions in using the application of the SSB Mobility in all respects.

SSB Mobility reserves the rights to revise or update these Terms and Conditions at any time. Your use of the Application constitutes your acceptance to the updated Terms and Conditions at the time.

1. Uses of Content or Technology on the Application

Data, artworks, messages, videos, audios, media files or images which are provided or included in the Application (collectively referred to as "Content") and software, technology, product, any related documents ("Technology") created by SSB Mobility, which are downloadable or accessible from the Application, are protected as copyrights, patents or trademark under the law of the Kingdom of Thailand, international treaties and the applicable laws of other territories.

Unless otherwise specified, you may access and use the Content or the Technology for the purpose of private use. You may not use the Content or the Technology for commercial purposes or for any purposes other than private uses. Furthermore, you may not duplicate, modify, distribute, offer for sale, or act in violation of technological measures applied to the Content or the Technology without authorization from SSB Mobility.

Technology made available on the Application may be subjected to a license in accordance with a separate license agreement which will be offered to you prior to downloading or accessing the Technology. Ownership and copyrights to the works relating to the Technology and any copies of the Technology are the property of SSB Mobility. You may access or download and use the Technology only when you have accepted and agreed to be bound by the provisions of the license agreement applied to the Technology or your use of the Technology. Any reproduction or re-distribution of the Content or the Technology or violation of technological measures in the Application without authorization from SSB Mobility is prohibited by law and may result in serious civil and criminal penalties. You may not carry out reverse engineering, disassemble, decompile, modify, and dissect any Content or Technology. You are prohibited to carry out any means to obtain the source code. You agree not to willful conduct any actions with or without any devices or use any computer software or transmit any virus to interrupt or intervene or disrupt the process of the service. Violators will be prosecuted to the full extent of the law. Use of services in the Content or Technology without expressed authorization by these Terms and Conditions or the separate agreement given by SSB Mobility constitutes a violation of these Terms and Conditions and may constitute an infringement of copyrights, patents, trademark or a violation of other laws. All rights for which expressed authorizations were not given herein are reserved by SSB Mobility. If you violate these Terms and Conditions, authorization to use the Content and the Technology will automatically terminate and you shall destroy any copies, materials which you may have acquired from any parts of the Content or the Technology immediately.

2. Application User

This Application is designed for persons who are 12 years and older. However, persons who are 60 years and older should use this application under supervision of family members.

The use of this application for the person who experiences Intellectual disabilities, developmental delays, unusual response, must be under guardianship.

3. Available Application

SSB Mobility intends to provide this Application for convenience of services only. SSB Mobility may collect data or control Application from networks or servers of SSB Mobility and SSB Mobility is not bound by any medical secrecy obligation of the user.

The information of doctors, infirmaries, medications, doctor's working hours, service queues, appointments, medication history, drug allergy history, laboratory results, and medical information of the patients are only references from the doctor, the hospital, or the infirmary only. This information may be erroneous or an update has not occurred. You acknowledge that Application cannot guarantee uninterrupted, continuous access to the services due to internet failure, other technical problems and other computer software threats interrupting the system. The application, including the hospital and the infirmary, intends to present the most accurate information to the customers. However, there may be some mistakes, incompleteness, inaccuracy, or error from the application or the database. Therefore, in using the service of the application, the customers agree, understand, and are aware of the risks that may occur from a mistake or misunderstanding agree to waive any legal proceeding and all damages from SSB Mobility, SSB affiliate as well as the hospital ,the infirmary ,doctors , pharmacists, and medical professionals in all cases.

This Application is made available for displaying services information, receiving of services, history of care, and medication information between you and the Hospital only. This application is not intended to be provided for providing advice or medical consultation or diagnosis and has no part in any interpretation or advice whatsoever. The Application only displays data which has been read by the Hospital and each of which may have different interpretation standards.

By using this Application, you consent for the Application to send notification, call for service queue, first three position approaching queue, medical appointment, alert for medication time, hospital's newsletter (collectively referred to as "Notifications") to your mobile devices or other communication devices.

You acknowledge that the Application is only a part of queuing administration by the medical personnel. Calling for treatment queue or services queue display in the Application may not reflect the actual order of calling which is at the medical personnel's discretion, and that the alarm for medication time and medication schedule are sent from the Hospital. This Application only facilitates displaying of information and warning in accordance with the time and date predetermined by medical doctors or pharmacists or medical professionals of the Hospital.

This Application is designed to allow you to gain access by using a password for the benefit of your privacy data protection. SSB Mobility recommends that you keep your password secretly and regularly change your password. SSB Mobility, Affiliate, Hospitals, and infirmaries shall not be held responsible whatsoever should you allow other parties to gain access to the Application or make a password to gain access to the application available to other parties.

This Application will be operated upon installation on your mobile devices or other communication devices connected to internet. The Notifications or any update may be delayed or stopped or cannot be reached depending on the quality of internet signal and your mobile devices or other communication devices. You acknowledge that turning off the Application or Notifications system on your mobile devices or other communication devices will prevent you from receiving the Notifications in any case and may result in the faulty or delayed Notifications. Application may not support medication notification, queue notification, particular services in particular mobile device. You acknowledge and agree that using application is at your own discretion. SSB Mobility, Affiliate, Hospitals, and Infirmaries shall not be held responsible whatsoever.

You agree and are aware that the Application is only the media that refers to the available or shown information from the database of the hospital or the infirmary where you have received the service. There may be some mistakes in the information or the information is not updated. The customers agree not to refer to the information from the Application only.

The application user agrees to check with the hospital staff at all times and in all cases about the medication information, the appointment, the laboratory information, the services, and your personal health profiles and also agree that there may be some errors from the application or non-updated database or a mistake that could lead to an inaccuracy of the medical information and the doctor information including working hours. You agree not to refer to this information only from the application.

You accept that particular hospital or infirmary may restrict the number of appointments scheduled and number of no-shows. If you are unable to keep your appointment, you are required to cancel/reschedule the appointment 1 day in advance of the appointment. You may not be eligible to schedule new appointment if your number of no-shows are exceeded the permitted number of no-shows. You may contact your preferred hospital or infirmary to enable you to schedule appointments. If you are late for appointment, you may be treated as walk-in.

The customers agree that the application may ask you to specify personal information to identify your identity. For example: the e-mail used for registration; the telephone number used to confirm the appointment, and the information of rights that you choose, including health care insurance rights and identification number.

SSB Mobility intends to provide you with a modern experience and convenience in receiving the medical services. You have given your consent and accepted that the application shall process your information for the objectives of providing the services and improving the quality.

SSB Mobility may share the information with affiliate, hospitals, infirmaries, or medical partners in order to improve the quality and respond to the needs regarding the experience that customers will receive to recommend technology improvements and improve the health benefits to customers.

You agree to keep good care of your medical and service information and communication devices used to record the medical history. If your communication device or password is lost and other people can gain access to your information, SSB Mobility and affiliate will not be able to delete the information that was put into your communication device.

You agree that if you delete the application from the device or sign out from the application, the information that you recorded will be lost. SSB Mobility and affiliate will not be able to retrieve that information.

The customers agree that the application is only a medium that provides information interpreting your laboratory results in the application for the convenience following up on your health history. You shall confirm the laboratory results with the doctor or medical staff or nursing staff at all times.

The application shall not disclose the laboratory results that cannot be disclosed to the application and it shall not input non-numerical information into the application. If there is any inaccurate information or if you have any questions about any of the laboratory results, please ask the medical or nursing staff from the hospital or the infirmary where you received the diagnosis or the information.

The customers agree that SSB Mobility, SSB affiliate, the hospital, the infirmary, and the medical and nursing staff shall not be responsible for the customer or anyone damaged from using the application service or from an error of the application or information from the database or mistake in the doctor's prescription or medical record or any other causes that may arise directly or indirectly from using the application or any services of the application.

This Application is designed for standard time of Thailand (GMT/UTC + 07:00 hour) and this Application does not, in any way, support Notifications while travelling in a different time zone in all cases.

You should regularly upgrade to a new version of the Application. Deleting, re-installing, upgrading to a new version of the Application may result in loss of previously recorded data.

SSB Mobility is located in Nonthaburi, Thailand. SSB Mobility makes no representation and warrant that the Content or the Technology of this Application is suitable and is downloadable outside of Thailand. Accessing the Content or the Technology may not be lawful for certain groups of persons or in certain territories. If you are accessing the Application outside of Thailand, you accept that such action is at your own risk and you are responsible for compliance with the law of that territory.

4. Personal Information

For the purposes of these Terms and Conditions, Personal Information means health information, use of services information, receipt of services information, treatment history, medication information, medical appointment between you and the Hospitals or Infirmaries.

Your personal information and the content appearing on the application are data which are recorded only by you and the hospital. SSB Mobility, Affiliate, Hospitals, and Infirmaries are not responsible for any content of personal information in any respect and does not warrant accuracy of such content of personal information. If you have any questions in regard to the Personal Information sent to your mobile device or other communication devices, please directly contact the Hospital or Infirmary which provided the data to you for further information.

You are responsible for recording, revising, adding, updating or deleting Personal Information which is given by you and you acknowledge that such revising, adding, updating or deleting of Personal Information by you may cause changes to your Personal Information and the Content on the Application or cause the Personal Information and the Content on the Application to be inaccurately displayed, which SSB Mobility shall not be held responsible for such change or deviation of data.

You agree that there may be some errors from the application or non-updated database or a mistake that could lead to an inaccuracy of your medical information displaying on the application. You agree not to refer to this information only from the application. You are aware of the risks that may occur from a mistake or misunderstanding and agree to waive any legal proceeding and all damages from SSB Mobility, Affiliate, Hospitals, Infirmaries, doctors, pharmacists, and medical professionals in all cases.

The customers agree to keep the personal information, which are the password and e-mail used for the registration, including OTP (One Time Password) and the passcode for the application confidential and to not disclose them to anyone.

You agree and accept security measures given to your Personal Information and medical data relating to health care to which you have consented to be displayed on your mobile device in all respect. If a third party gains access to your Personal Information, SSB Mobility, Affiliate, Hospitals, and Infirmaries shall not be held responsible or liable in any circumstances.

5. Advice and Specific Information on Medication

Medication information, mode of medication delivery, forms, dosages, and medication schedule correlate to what the medical doctor and pharmacist have examined and prescribed to you. The Application only serves as an electronic media which displays the data from the Hospital or Infirmary. The Application has no part in formulating, calculating and making any decision whatsoever.

Before using the Application on medication schedule, you are advised to always confirm the first date of taking the medication specified in the Application against the first date and time of taking the medication specified on the label of the medication or as prescribed by the medical doctor or pharmacist at the Hospital or Infirmary prior to taking the medications. You acknowledge and agree to define dinning meal time with truth at the service kiosk. You acknowledge that the meal times defined are essential part for dosage time displaying on the application.

You acknowledge and agree that taking the medication is at your own discretion. This Application only displays the information as per the instructions of the medical doctor or pharmacist. SSB Mobility and affiliate shall not be held responsible in any way should the medication dosages not correspond to the medication schedule as prescribed by the medical doctor and the pharmacist.

Do not rely solely on application on your decision to take medications.

The customers are aware, agree, and understand that before taking the medication at all times, the users shall not refer to the oral medication, the information for the oral medication, or the information that is shown, appears, or reminded through the application only. The customers shall examine the accurateness of the medication information input and how to take the medication at all times with the pharmacist before taking the medication. The customers shall examine the accurateness of the reminder for taking medication and how to take the medication before taking the medication at all times. SSB MOBILITY, Affiliate, Hospital, Infirmary, Pharmacist, Doctor shall not be responsible for the problems that occur from taking the medication in all cases, including all damages that may occur from an error of information and application.

If you missed the medication or take the medication but not at the accurate dosage as per the medical doctor's prescription, please immediately consult your doctor.

If you feel any irregularities after taking the medication, please immediately consult your doctor or pharmacist.

6. Advertiser and Advertising Network

SSB Mobility may provide advertisement on the application. Certain advertisements may be operated by SSB Mobility and affiliate while other certain advertisements may be operated by a third party. Certain advertisements may provide a link to a website or a portal operated by the advertiser or other third parties who may have a relationship with the advertiser. Once you have clicked on the advertisement, SSB Mobility and affiliate are no longer responsible nor have any control over the procedure, policy with respect to handling of Personal Information by the advertiser. When and if the advertiser requests your Personal Information, you should always check the advertiser's policy on Privacy Statement.

Advertising network and the third parties may use cookies, web beacons, and other technologies to collect data other than Personal Information or history of visiting various websites other than the Application by SSB Mobility. SSB Mobility has no control over such said cookies, web beacons, and other technologies implemented by the third party.

7. Governing Law

These Terms and Conditions are construed by and governed in accordance with the law of the Kingdom of Thailand. Without regard to the conflict of rules of law, if any of the provisions or clause of these Terms and Conditions is deemed to be unenforceable or invalid by any courts which has full jurisdiction over the matter, such invalidity of provisions or clauses of the Terms and Conditions shall not impact the validity of the surviving provisions or clauses of the Terms and Conditions which would be considered to be enforceable in full and there shall be no waiver on any provisions or clauses of these Terms and Conditions which constitutes a waiver on the next or any other subsequent provisions or clauses.

8. Feedback

Comments or any other materials sent to SSB Mobility including feedback such as questions, comments, suggestion or any others of the same nature (collectively referred to as "Feedback") shall not be regarded as confidential. SSB Mobility, affiliate shall not be held liable relating to the Feedback and are able to use, disclose freely for development of the Application. Moreover, SSB Mobility and affiliate may use ideas, innovation or techniques contain in the Feedback for the benefit of SSB Mobility which includes and not limited to research and development, production and marketing of products which may incorporate such Feedback.
